Alexander has been out since early September with back spasms. His back is still bothing him, so the out-of-contention Orioles won't press the issue. Alexander was limited to 93 games this season due to injury but was a valuable super utility player when on the field, slashing .303/.358/.414 with four home runs and 10 stolen bases. The 27-year-old will likely have a similar role with the Orioles in 2027.

Bradish had already been ruled out for the remainder of the season after hitting the shelf Sept. 13 due to a sports hernia, so his move to the 60-day IL is merely procedural. The transaction clears a spot on the 40-man roster for right-hander Albert Suarez, who had his contract selected from Triple-A Norfolk.

O'Neill had already been ruled out for the remainder of the season with a strained right rotator cuff, but the goal of the injection is to speed along the healing process. He's fully expected to be ready to go in time for spring training. O'Neill slashed just .212/.300/.377 over 85 games for the Orioles this season, and he is set to make $16.5 million in 2027 in the final year of his three-year, $49.5 million contract.

Basallo had to be removed from Monday's game versus the Blue Jays after taking a foul tip off his catcher's mask, and testing has determined he suffered a concussion. It will end the season of Basallo, who has also dealt with a nagging right shoulder injury during the second half. The 22-year-old will finish the 2026 campaign with a .222/.291/.406 batting line and 18 home runs over 112 contests. While Basallo is out, Carlos Narvaez will serve as the Orioles' primary catcher in the final days of the regular season.

Holliday is dealing with a triangular fibrocartilage complex tear in the wrist. The current estimate points to Holliday needing 10 weeks to rehab, which would give him plenty of time to be ready ahead of spring training. Holliday was limited to only 98 games this season due to injury, slashing .229/.300/.340 in those contests.

Helsley has been working his way back from right elbow inflammation that forced him onto the injured list in late June, but he hasn't pitched in a rehab game at Triple-A Norfolk since last Saturday. With his progress stalling, the Orioles have officially now shifted their focus to getting the 32-year-old ready for 2027. He'll finish 2026 with a 4.11 ERA and 1.43 WHIP alongside a 21:9 K:BB across 15.1 innings.

Mountcastle initially went down with a left foot fracture in mid-April, but it's an oblique strain he suffered in late July that's been the more recent cause of his continued absence. A return before the end of the month remains possible, but even if that happens Mountcastle's role could be relatively limited.

Robert's absence from the starting lineup for a second straight day in Sunday's series finale versus the Red Sox seemed to hint that the veteran outfielder was nursing an injury, and the Orioles confirmed as much about an hour before first pitch. Since Robert's placement on the IL is retroactive to Saturday, he'll be eligible for activation Sept. 15, though it's not yet clear if he has a realistic chance at returning that day. After being claimed off waivers from the Mets on Aug. 31, Robert went 2-for-11 with a home run, a double, a walk, three RBI and two runs through his first three games with Baltimore.

He's been shelved for the entire 2026 and opted for surgery following multiple opinions. The procedure will knock Selby out for the remainder of this season, and he's expected to get a late start to next season, as well.

Dr. Keith Meister performed the surgery after Akin sought out a second opinion on his left elbow. He was originally diagnosed with inflammation, but the problem was obviously much worse. Akin will miss the remainder of the 2026 season and likely most of the 2027 campaign. The 31-year-old appeared in 24 games (two starts) for the Orioles this season, logging a 5.68 ERA, 1.50 WHIP and 17:9 K:BB across 25.1 innings.

Westburg's procedure will keep him out for the remainder of the 2026 campaign, but general manager Mike Elias said Friday that the 27-year-old infielder has a good chance to return to playing in the field full-time by early 2027. Until then, Coby Mayo will likely take over as Baltimore's primary third baseman, though Jeremiah Jackson, Blaze Alexander and Weston Wilson are also candidates to make an occasional start at the hot corner.

The move corresponds with Alexander Cornielle being promoted from Triple-A Scranton/Wilkes-Barre ahead of the second game of Friday's doubleheader against the Orioles. There's a chance Cruz returns to action if the Yankees make it to the World Series, though it's more likely that the right-hander is shelved for the rest of the 2026 season due to a partial UCL tear in his right elbow.

Weathers will be facing fellow rehabbers Giancarlo Stanton (calves) and Trent Grisham (hamstring) in the session. After completing his recovery from an internal brace procedure he underwent in July of 2025, Schmidt returned to the Yankees and pitched a scoreless two-thirds of an inning against Minnesota on Sept. 16. However, he suffered a biceps injury while warming up in the bullpen four days later. The right-hander reportedly had a "better" day Tuesday, but it's still unclear if he'll be able to return before the end of the season. Should Schmidt not be feeling well enough to pitch by Thursday, the Yankees may decide that he's better off being held out of pitching until 2027.

Grisham is said to be "pretty deep into his running progression," which is the most significant part of his recovery from a right hamstring strain. He's also been hitting and doing other baseball activities. Grisham has already been ruled out from returning to action during the regular season, but if he continues to progress, he could be activated at some point during the playoffs.

Judge will miss the rest of the regular season due to a moderate right calf strain, but the fact that he's making an effort to play right away in the postseason is an encouraging sign of progress. However, the slugger's comments directly contradict manager Aaron Boone's belief that Judge may not be available until the ALDS. More clarity regarding Judge's return timeline will likely emerge after a few more days of recovery.

Stanton will close the regular season on the 60-day injured list after straining his left calf in August while rehabbing a right calf strain. Despite the ongoing injury issues, it sounds like Stanton could serve as the designated hitter or perhaps as a bench bat during the first round of the playoffs. Aaron Judge (calf) is not expected to be ready for the wild-card round.

The transaction frees a spot on the 40-man roster for the addition of Michael Fulmer, who signed a one-year contract. Castro hit the shelf in mid-August due to right elbow neuritis, and Gary Phillips of the New York Daily News reports that the 27-year-old is scheduled to meet with Dr. Keith Meister in the near future to determine whether surgery is necessary to address the injury.
